Details of health entity (EPS) and supporting documents documents

Details of pension with affiliation number (AFP)

Social security affiliation form

Copy of Cédula (ID)

Copy of Visa (if applicable)

Copy of military notebook (if applicable)

Medical examination completed

AFC - Optional pension. Provider, website, account details and signed contribution letter.
